Report Guidelines - Please submit the medical reports you have and your questions for the experts. You can also mention your treatment preferences, such as a desire to avoid surgery, and travel or financial constraints. Our experts take this into account when recommending treatment. • Physician Medical Summary form detailing your condition and treatment - Planned treatments like your current surgery plan, chemotherapy protocol, radiation therapy (type and site), etc.
